Admissions & Assessments

Creating a smooth, safe and personalised
transition into our care.

At Crestos Care, we believe that every successful placement begins with a thorough understanding of the individual. Our admissions process is designed to ensure that every person receives the right support from the very beginning.

Each referral is carefully reviewed by our management team to ensure we can meet the individual's needs safely and effectively. Where appropriate, we carry out a comprehensive assessment involving the individual, their family, current care providers and multidisciplinary professionals.

Following assessment, we develop a personalised care plan, support plan, risk assessment and, where required, relapse and crisis management plans. We also work closely with commissioners to develop a smooth transition plan, allowing individuals to become familiar with our service before moving in.

Our goal is to provide a safe, well-planned and person-centred transition into care.

Promoting healthier lives through accessible,
person-centred healthcare.

We work closely with local healthcare professionals to ensure every individual has access to the medical support they need.

We maintain strong relationships with Community Learning Disability Teams (CLDT), local GP practices and NHS services. Every individual is supported to register with a GP, dentist and optician, while also receiving annual health checks and personalised Health Action Plans.

Where additional support is required, we ensure reasonable adjustments are made so that everyone can access healthcare safely, comfortably and with dignity.

Our aim is to promote long-term physical wellbeing by ensuring every individual receives the healthcare, guidance and ongoing support they deserve.

Education & Employment

Encouraging independence through learning, development and opportunity.

We believe everyone should have opportunities to learn new skills, build confidence and achieve personal goals.

Individuals are supported to access local colleges, adult education, vocational training and voluntary opportunities. Where appropriate, we also assist people in preparing for and accessing paid employment.

Alongside education and employment support, we encourage the development of everyday life skills including literacy, numeracy and independent living skills, helping individuals reach their full potential.

By promoting lifelong learning and meaningful opportunities, we empower the people we support to live more independent, fulfilling lives.

Supporting meaningful experiences that enrich everyday life.

Meaningful activities are an important part of physical, emotional and social wellbeing.

Every individual works alongside their care team to create a personalised weekly activity programme based on their interests, goals and preferences. Activities may include community outings, hobbies, therapeutic activities, exercise, social events and leisure opportunities.

We believe financial circumstances should never become a barrier to participation and always encourage choice, inclusion and community involvement wherever possible.

Our focus is on helping people build confidence, develop relationships and enjoy fulfilling experiences that support both independence and wellbeing.

At Crestos Care, the people we support are involved in every stage of planning and reviewing their care.

We encourage open communication through regular meetings, care reviews and daily conversations with our care teams. Families, advocates and healthcare professionals are also involved where appropriate.

By listening to the people we support, we ensure that our services continue to reflect their wishes, goals and changing needs.

Working collaboratively allows us to build trusted relationships and provide care that is responsive, personalised and focused on achieving positive outcomes.

Everything we do is centred around the individual.

We encourage people to make informed decisions about their own lives and provide information in accessible formats to help them understand their choices. Communication passports and Easy Read materials are available where needed.

Care plans are developed with the individual rather than for them, ensuring they remain in control of decisions wherever possible. Reviews are carried out regularly so that support continues to reflect changing needs and personal aspirations.

By promoting independence, choice and empowerment, we help individuals take an active role in shaping the support they receive.

At Crestos Care, we recognise that spiritual wellbeing can play an important role in a person's overall health and quality of life.

As part of our assessment and care planning process, we take time to understand each individual's religious beliefs, spiritual preferences and cultural practices. We encourage and support people to practise their faith in ways that are meaningful to them, whether that involves attending places of worship, participating in faith-based activities or maintaining personal spiritual routines.

We actively encourage comments, suggestions and feedback from the people we support, their families, advocates, healthcare professionals and commissioning authorities. Regular reviews, service user meetings and ongoing communication with key workers provide opportunities for individuals to share their experiences and help shape the services we deliver.

Should concerns arise, we operate a clear, transparent and accessible complaints procedure. Every complaint is treated with respect, investigated thoroughly and responded to fairly and promptly. Where appropriate, lessons learned are used to improve our services and ensure we continue to deliver safe, high-quality, person-centred care.
